Initial Information Collection for Environmental Impact Assessment

Purpose

This questionnaire is designed to gather preliminary information about your project to help determine the appropriate environmental assessment category and scope of study. Accurate completion of this form will expedite the assessment process and ensure compliance with all regulatory requirements.

SECTION 1: PROJECT IDENTIFICATION

Instructions: Provide complete information about your proposed project
